师范类：往届毕业生需准备以下材料：
1、本人身份证原件和复印件；
2、师范教育类专业毕业证书原件和复印件；
3、申请人思想品德鉴定表（在职人员由其所在单位提供，非在职人员由其户籍所在地乡（镇）政府或街道办事处提供）；
4、本人填写教师资格认定申请表；
5、由教师资格认定机构指定的县级以上医院出具的体格检查合格证明；
6、普通话水平测试等级证书原件和复印件。应届师范生可在其毕业前的最后一个学期，向就读或拟任教学校所在地教师资格认定机构提出认定教师资格申请，申请时以学业成绩单代替学历证书，思想品德鉴定表由在读学校提供。其余要求与往届生相同。上述材料经教师资格认定机构审查合格后，方可领取教师资格证书。非师范类：与师范类应届、往届毕业生需准备的材料基本相同。只是在初审合格后，须参加由教师资格专家审查委员会组织的教育教学基本素质和能力考察，并提供教育学、心理学考试成绩证明。如果符合条件，教师资格认定机构将在相应的受理期限内通知认定结果。
（二）申请时间：教师资格认定机构和依法接受委托的高等学校，每年春季、秋季各有一次受理资格认定时间。考试课程及时间非师范毕业生（包括在职人员和非在职人员）：考试课程按所报类别分为：高级教师开考高等教育学、高等教育心理学；
中学（含中等职校）教师开考教育学和中等教育心理学；
小学教师开考教育学和小学教育心理学；
幼儿教师开考幼儿教育学和幼儿心理及卫生学。每年开考一次。
（三）认定机构：根据有关规定，县级教育行政部门负责受理幼儿园、初级中学教师资格认定；
地（市）州级教育行政部门负责受理高级中学教师、中等职业学校教师及其实习指导教师资格认定；
省级教育部门负责受理高校教师资格认定.)属非师范教育类专业毕业生的，现场确认应提交下列材料：

2014 届师范类毕业生教师资格认定工作于3月10日正式启动。流程如下：
1、3月10日--21日。组织所有参加认定学生体检。
体检单位：咸宁市中心医院体检中心体检费150元。
学生以班为单位将体检费缴纳到账户 建行 廖龙英 6217 0027 4000 1221 830 凭缴款回执到鉴定中心办公室领取体检表。
2、4月（具体时间见中小学和幼儿教师教师资格考试网）网上申请教师资格认定
3、5月现场确认 确认地点：咸宁市行政服务中心
4、6月统一上交毕业证书复印件地点：温泉校区行政楼1016室
5、7月初统一办理教师资格证书邮寄 邮寄费用23元 需要办理的同学以班级为单位统一缴费至建行 管俊 6227 0027 4601 0427 044凭汇款回执在鉴定中心办公室领取邮寄封套填写地址。不要要办理邮寄的秋季开学后在行政楼1016室凭身份证领取证书。
说明：2014年以后办理教师资格证书不收取任何费用。2010年在新生入学时已缴纳200元体检费用的学生，由班级统一办理手续，每人退款50元。

关于做好2010年中等职业学校
中小学幼儿园教师资格认定工作的通知
各市教育局，有关高等学校：
根据•中华人民共和国教师法‣（以下简称•教师法‣）、•教师资格条例‣、•†教师资格条例‡实施办法‣和•山东省实施细则‣（鲁教人字„2001‟22号）的有关规定，现就做好我省2010年中等职业学校、中小学和幼儿园教师资格认定工作通知如下：
一、认定范围
幼儿园、小学、初级中学、高级中学、中等职业学校教师资格和中等职业学校实习指导教师资格的申请人员，包括：
（一）各级各类学校已取得教师资格拟申请其他等次教师资格申请人员；
（二）2010年普通高校应届毕业生；
（三）其他符合•教师法‣规定学历条件的人员。
二、申请条件
（一）未达到国家法定退休年龄、户籍或工作单位在我省的中国公民。
（二）要具备•教师法‣、•教师资格条例‣及其•实施办法‣规定的相应学历。
（三）教育教学能力应当符合下列要求：
1.取得•山东省教师资格认定教育学、心理学考试合格证‣(以下简称•合格证‣)并经教师资格认定机构进行教育教学能力测评合格（修学教育学、心理学课程成绩合格且参加毕业教学实习成绩合格的普通院校师范类毕业生可免测）。
2.普通话水平要达到普通话水平测试二级乙等以上标准，取得相应合格证书。
3.适应教育教学工作的需要，具有良好的身体素质和心理素质，无传染性疾病，无精神病史，并经教师资格认定机构组织体检合格的。
（四）要遵守宪法和法律，遵守教师职业道德，能履行•教师法‣规定的义务，经申请人所在单位（含就读学校）或户籍所在地乡镇人民政府（街道办事处）思想品德鉴定合格。
三、认定程序
（一）教育学、教育心理学考试
申请教师资格认定人员须到户籍或工作单位、就读学校所在地教师资格认定机构报名参加教育学、教育心理学（以下简称“两课”）考试，取得•合格证‣。
（二）教育教学能力测评
取得•合格证‣的申请人员参加市、县（市、区）教师资格认定机构组织的教育教学能力测评。
（三）体检
取得•合格证‣且教育教学能力测评合格的申请人员和符合教师资格认定特许条件的申请人员，由教师资格认定机构按•山东省教师资格认定体检标准及操作规程‣组织体格检查。
（四）资格认定
符合条件的申请人员填写•教师资格认定申请表‣，提供身份证件、学历证明、•合格证‣、•山东省申请教师资格人员体格检查表‣、•普通话水平测试等级证书‣及•申请人思想品德鉴定表‣，向户籍或工作单位、就读学校所在地的教师资格认定机构提出申请。
教师资格认定机构受理申请后在30个工作日内作出认定结论。认定合格者颁发•教师资格证书‣，按要求将文字材料归档保存，并使用“全国网络版教师资格认定信息管理系统”录入认定数据信息。
普通院校师范类毕业生修学教育学、心理学课程成绩合格且参加毕业教学实习成绩合格的，可免测以上
（一）、（二）项测试程序。但学习期间缺少教育学或心理学合格成绩的，仍须参加“两课”考试并取得•合格证‣；
缺少毕业教学实习合格成绩的，仍须参加教育教学能力测评且成绩合格。普通高校师范类
应届毕业生的教师资格认定工作，由各高校学生处或就业指导中心负责统一组织，参加驻地市教师资格认定机构的认定。教师资格认定机构要确保毕业生离校前向合格人员发放•教师资格证书‣。
四、“两课”考试
2010年“两课”考试由我厅统一组织，分中学（含中等职业学校）、小学和幼儿园3个层次。申请认定中等职业学校教师资格，中等职业学校实习指导教师资格，高级中学、初级中学教师资格的人员参加中学层次考试；
申请认定小学教师资格的人员参加小学层次考试；
申请认定幼儿园教师资格的人员参加幼儿园层次考试。
考试报名工作采用网上报名与现场报名相结合的方式进行。考生须在规定时间内访问山东省教师资格认定指导中心网站（）公布。
（三）体格检查
教学能力测试合格对象和免于测试对象都必须参加统一组织的体检，体检人
员名单及体检工作安排于教学能力测试结束一周内公布在仪征教育信息网
（.cn），体检结果在体检结束10日内公布在仪征教育信息网，体
检合格人员在规定的提交材料时间内提交材料。
（四）提交材料
1、提交材料时间：公布体检结果时一并公布
2、提交材料地点：仪征市行政办事服务中心综合窗口
3、提交材料种类：
（1）申请认定教师资格材料袋封面（现场领取材料袋并粘贴）；
（2）《教师资格认定申请表》2份(正反打印)；
（3）身份证复印件；
（4）户籍证明材料复印件；
（5）符合相应条件的学历证书复印件（2010年应届非师范毕业生须提交学
业成绩证明原件，样式附后）；
（6）普通话水平测试等级证书原件、复印件；
（7）思想品德鉴定材料1份；
其中在职人员应由所在工作单位提供思想
品德证明。应届毕业生应由毕业学校所在院系党组织提供思想品德证明。非在职
人员应由其户籍所在地街道办事处或乡级人民政府提供思想品德证明，并由当地
公安部门 提供无犯罪记录证明；
（8）非师范专业学历提供教育学、教育心理学补修合格证原件；
无法以学
历证书确认师范身份者需提供学历证书颁发学校该专业的招生考试说明和教育
实习档案复印件（档案管理单位需加盖公章并注明与原件相符）；
已具有初中、高中、中职、中职实习指导教师资格证书的提供证书复印件；
（9）粘贴相片页。
复印件请统一用a4纸按封面的证明材料顺序装订好。
（五）证书发放
经认定符合教师资格条件的人员名单在仪征教育信息网公布，于7月初到仪
征市行政办事服务中心综合窗口领取教师资格证书。
